Vision Benefits (VSP)

VSP provides you with the advantage of group discounts for services and products, such as vision testing and eyeglasses. VSP enrollment is optional, and you pay the entire cost of the premium.

To take advantage of VSP's discounts, you must use VSP network providers. VSP's network providers include ophthalmologists and opticians, as well as affiliate providers, such as Costco, Visionworks, and other retail chains. To find network providers, call 800-987-7195 or visit www.vsp.com. Please note, for the eye exam benefit provided by your DMBA medical plan, you must use a DMBA network provider.

Enrollment

To learn more about your vision plan and check your current enrollment, log in to www.dmba.com. Navigate to My Plans and under Benefit Summaries select Vision Plan. Please remember:

• If you're currently enrolled, your enrollment will carry over automatically next year.
• After you enroll, your effective date of coverage is the first day of the following month.
• Your VSP ID number is your DMBA ID number. VSP does not provide a separate ID card.

Your Benefit Options

VSP provides two benefit options. Both options include glasses or contact lenses. One option also includes an annual eye exam, while the other does not.

Please keep in mind that your VSP plan must match your medical plan. For example, if you have a family medical plan, you must have a family VSP plan. To reach VSP, call 800-877-7195.
