[ Volunteer Station ]

We appreciate your willingness to serve here at BYU-Hawaii. Before you can begin your services and be issued an I.D. card you will need to complete three simple steps:

Complete and sign "Liability Release and Assumption of Risk for Volunteers" Volunteer Data Sheet TB Testing (Note: The TB test can be taken on campus at Health Services (free of charge). However if you miss your first reading, you will be charged for a second test.When you arrive on campus, submit forms to Human Resource Services to receive an "Employee Identification Certificate" which you will take to the BYUHSA office to obtain your I.D.
