Health Assessment Now Available!

January 1 - December 31

The Health Assessment required for all employees participating in the DMBA Incentive Program is now available online.

Please use the following instructions for filling out the Personal Health Assessment (PHA)

1. Click the following link to access the Assessment:
   • https://wellsuite.com/dmba/pwp/approot/PWPTermsOfUse.asp?sid=2007010807381820168
2. Read the Terms of Use page and press “I Agree”
3. Login on the site by using your DMBA ID number and password. NOTE: If you are a new user click the “New User?” link and fill out the information.
   • DMBA members: Your Deseret Mutual ID Number is the 9 digit number on your insurance card that begins with 00…
   • Altius members: Your Deseret Mutual ID Number is your Social Security Number.
   • Spouses and Children: Your Deseret Mutual ID Number is your Social Security Number.
4. Fill out the questionnaire. You will have a maximum of 30 minutes to complete the assessment. If you run out of time or need to finish the questionnaire later, click the “Save” button to save the answers you have completed so far.
5. Click on the “Generate Report” button as soon as you’re finished, to save your answers, compute your scores and produce your report.

NOTE: If you have had the following tests or measurements done recently, have the results on hand to help you in filling out the assessment. (Otherwise omit this section).
