BYU Hawaii representatives will help you plan for retirement by requesting retirement estimates from DMBA and providing information regarding the Master Retirement Plan, supplemental retirement plans (DMBA Thrift Plan and TIAA/CREF), Social Security, Medicare, medical insurance, life insurance, and other university benefits after retirement. How do I take out a loan against my DMBA Thrift Plan or TSA?
Call the DMBA Savings Plan Phone System at 1-800-777-3622 and follow the steps through the automated phone system to request that the paperwork be sent to you. Online loan applications are also available at dmba.com.
How do I make a withdrawal from my DMBA After-tax Retirement Savings account?
Call the DMBA Savings Plan Phone System at 1-800-777-3622 and follow the steps through the automated phone system to request that a withdrawal form be faxed or mailed to you or login to dmba.com .
What do I do if I have a name change?
Bring your new social security card to the Human Resource Office and fill out a name change form. If you are a former student, submit your name change form to the Registrar's office.
How do I add my new baby to the insurance?
You must fill out an add form at the Benefits Office within 60 days of the baby's birth or add your new dependent online at dmba.com .
How do I add my new spouse when I get married?
Bring a copy of the marriage certificate to the Human Resource Office and complete the enrollment form within 60 days of the marriage or online at dmba.com .
How long are my dependents eligible for DMBA medical, dental, and life insurance?
Dependents are eligible for DMBA medical, dental, and life insurance until marriage or age 26 if they are a student, missionary, or not employed with benefits.
Who do I contact about benefits regarding the death of a family member?
Please notify the Human Resource Office immediately following the death of a family member covered by benefits. When you have received death certificates, the Human Resource Benefits Manager will assist you in processing the necessary paperwork.
How do I change my primary care physician?
Call DMBA at 1-800-777-3622 to change your primary care physician or online at dmba.com. Because DMBA contracts with physicians on a monthly basis, the change will take effect the first day of the following month.
How do I make savings and allocation changes to my DMBA Thrift Plan account?
All changes, including contribution percentages and investment allocations must be made by accessing the DMBA Savings Plan Phone System, call 1-800-777-3622 and follow the steps through the automated phone system. Or use the DMBA website at dmba.com .
What happens to my benefits when I go on leave?
If you receive a University-approved leave (i.e., professional development, military service, ministerial service, illness, etc.) you should contact the Human Resources Benefits Manager to ensure that benefits will continue as desired and that proper premiums will continue to be paid.
What are the eligibility requirements for my child's tuition benefit?
If you are a full-time employee, you must work for a year before your dependents are eligible for tuition at BYU. The benefit extends through your dependents' undergraduate degree, 152 credit hours (included credits taken from BYU Provo, BYU Idaho, or LDS Business College) or age 30, regardless of marital status.
What are the eligibility requirements for my child to get a BYUH ID Card?
Dependents of full-time employees are eligible for a BYUH ID card if they are at least 6 years old and under 30 years old and are unmarried. Those who receive ID cards agree to abide by the BYU Honor Code and Dress and Grooming Standards.
