Request for Tuition Benefit
1. Before completing this request, please read this form
to review University policy regarding class work. Requests to
use the tuition benefit that do not conform to policy cannot
be approved.
2. Completion of this request does not constitute registration
for this class. Registration must be completed in the usual
manner at the Registrar's Office.
3. In the event the University representative has signed the
tuition waiver in good faith assuming the applicant is eligible,
but later it is determined the recipient is no longer eligible,
it will be the responsibility of the individual to pay immediately
the further costs owing the University.
4. Please complete Part A for an employee taking
a class(es) or Part B for all others.

Note: In the event the dependent or employee
withdraws from a class or school and has received tuition waiver
benefits,
he/she may be financially responsible to pay
the University for the unused portion of the tuition benefit.

In order than job assignments or class work not
be compromised, it is University policy that faculty, administrative
and staff personnel may not be full-time students.
Eligible faculty, administrative staff and staff
may register for two courses per semester and one class per term
and will not be required to pay the regular tuition.
Only one class may be taken during their regular
work hours.
Those wishing to take a class during the daytime
hours of 7 a.m. to 5 p.m. must have written approval of their
dean/associate dean or supervisor.
Under no circumstances is more than one class to
be taken during the employee's regular work schedule, regardless
of days and times the class is offered.
Regular Faculty, Administrative,
and Staff Personnel
A faculty member who is not teaching and who is
not under contract to the University for research, administrative
or other special assignments during a Spring or Summer
Term may take as many credit hours as the person's
division or school will permit.
New faculty members may not take class work during
the Spring or Summer Term they begin their school year teaching
assignments.
Regular administrative staff and staff employees
are expected to maintain a forty-hour work week and keep current
with the make-up hours.
Spouse of a Regular Employee
After an employee has completed one year of regular
full-time service, the spouse of a regular employee (faculty,
administrative, staff) may register for courses of study with
no limit as to the number of hours, day or evening,
and will not be required to pay the regular cost of tuition.
Children of a Regular Employee
After an employee has completed one year of regular
full-time service, children of a regular employee (faculty, staff,
administrative) may register for courses of study and
pay one-half the cost of regular tuition. Benefits
for dependents cease when the following occurs:
The dependent reaches age 30, or
The dependent receives the BA/BS Degree, or
The dependent has taken 152 credits (including credits
earned from BYU, Ricks, and LDS Business College).
Temporary Employees: Part-time
or Full-time
Employees classified as temporary are not eligible
for tuition benefits.
Aloha Summer Session Classes
and Evening Classes
Tuition benefits may be available for credit classes
only during Aloha Summer Sessions or evening classes.
