The educational, social and religious programs at Brigham Young University-Hawaii Campus require an employee be committed to the behavioral, moral and spiritual standards of the Church of Jesus Christ of Latter-day Saints and willing to follow the "Code of Honor" and "Dress and Grooming Standards" of the Church Educational System.

Completed forms should be mailed to the Human Resource Services Office at BYU-Hawaii. The address is on the form.
